Childcare Registration
On Sunday nights, Monday nights and Wednesday nights childcare is provided for on campus classes and activities. Space is limited. Registration is required in order for your child to attend.
Please fill in the form below to register your child for evening childcare.
Also by filling out this form you give your consent to the following statement.
I give permission for my child(ren) to join the activities of Grace Point Church. I hereby release GPC, its staff and sponsors, from responsibility and liability for any injury or illness that my child may sustain during this activity. In the event of an emergency, I hereby authorize an adult leader of the activity, as agent for me, to consent to any x-ray, examination; medical, dental or surgical diagnosis; treatment; and hospital care advised and supervised by a physician, surgeon, dentist (as appropriate) licensed to practice under the laws of the state where the services are rendered, either at a doctor’s office or in any hospital. I expect to be contacted as soon as possible.
